Informational Form Design
The application form needed a new look, by reorganizing the structure, layout, overall unity, hierarchy, and typography it got the face lift it deserved. This open up folder allows potential parents to be excited to complete rather than intimidated. Most of the form is separated into two sections allowing for one or both guardians to include their input, while still providing sections for them to share the responsibility.
